You may print out and complete the registration form. Adults should fill out the Adult Registration Form; if the client is under 18 years of age, use the Youth Registration Form instead. Compassionate Care Fund Application
It is our desire to minister to people, regardless of their ability to pay. Therefore, we offer services on a "sliding scale" through our Compassionate Care Fund. You may download and complete the Compassionate Care Fund Application to apply for this assistance, or request the application from our office at 717-656-4834. Please submit the application, along with proof of household income to our office, in order to qualify for a reduced rate. The reduced rate will apply only to services received after the reduced rate has been approved. |
